Background :
  • -Passed the NREMT, (10/1)
  • -Completed live scans for both the state certification and DMV ambulance cert (10/2)
  • -Sent my paperwork to the state agency (10/3)

This is what I was given at the DMV for my ambulance cert (10/2) :
  • -Application for Ambulance Cert
  • -DL-51
  • -Live scan request

However, after doing a little bit of reading on here regarding the DMV and ambulance certification, I have found that the DMV has a reputation for not providing all the required forms :
Can anybody confirm?

Green cards are no longer used. Your medical evaluation form can not be completed by any doctor now. The doctors have to be approved thru the DMV to do the exam.

I have found a list of doctors approved by the DMV. My only wonder is, what do I bring for the doctor to fill out? Just the DL-51 or do I also include the medical examiner's certificate that I found on this thread, but never given at the DMV?

Green cards are no longer used.

Instead its a piece of paper like a receipt that the DMV issues. You have to carry it with your ADL at all times.

Your PCP should be able to sign off on your physical, you bring the "20 questions" sheet (DL-51) that you get at the DMV.

I see. My PCP is not registered by the National Registry of Medical Examiners, so I will need to find a examiner who is registered, and from my understanding... only bring a DL-51.

I was just asking to make sure so that I wouldn't have to go a second time to the doc just because the DMV forgot to give me this.
What is the medical examiner's certificate used for then?

I don't believe you have to carry the white sheet with you anymore like you did the green card.
You still do. It's a full sheet of paper so what most people do is shrink the document down to just the size of a credit card.

It shouldn't really be confusing at all. Especially in SOCAL, any truck stop Doctor that does the state or DOT physicals can do it. My doctor from my tow truck days still does it now for my ADL, same dl51 form. It's really quite simple.
