Hi,

Please consider the following scenario and let me know what I should do:

I arrive in a room (alone, with no one else and without any equipment) and see that a man is sitting on the floor with his back supported by a wall. He is alert at first but then faints suddenly and starts to fall sideways - that is, his back is still touching the wall.

What is the best way to stabilise the patient as well as maintain an open airway? What extra precautions to take if he has spinal/head injuries?

I think you should do your own homework!

Call the fire department.

Go into the room on the other side. If it's an exterior wall, have the ladder truck put up it's latter.

Cut through the wall.

Stabilize victim.

I think it might be helpful to put your sarcasm in italic or something. :)

BTDT - patient wouldn't fit through the door.

You shouldn't do anything. You have no BSI...

Why are you going into a house alone? Scene Safety...

;)

More importantly, don't forget about the rope, tennis balls, and a scary old lady...

/A scene from that was taped at my undergrad!

No good, moving the wall might disrupt the integrity of the spinal column. The only REAL choice here is to cut a hole in the roof, be lowered in via aircraft hoist, apply a LSP Halfback, perform a surgical cric to secure the airway (the only SURE way) and be hoisted back into the aircraft for transport to a level I trauma center "just in case" (preferably Shock/Trauma in Baltimore).

It's all about the mechanism don't you know!

This does fit the best for the situation. Just make sure you know how to do the cric. If not look up on youtube first.

I don't want to watch a 6 minute video, but I saw the mash episode where Father Mulcahy secured an airway with a pen tube, and a pocket knife. Is that good enough?

You should at least download the audio to your ipod so Hawkeye can talk you through it, should the need arise.

Nope! Father M did it with his trusty pin knife and a tube from an ink pen

Here's an idea, run to your closest home improvement store and buy all the expanding foam insulation in a can they have (you know the kind you seal around windows and stuff with) then put a mask on the patient with a really really long hose on it then fill the entire room with the insulation. Allow the insulation to set for a couple of hours then remove the building from around the block of insulation. Transport the entire room sized block of insulation to the ER on a flat bed then dare the ER doc to say something about not securing the C spine properly :) I can't believe they haven't already made this the national standard.

these are some really good ideas hahaha

Shock/Trauma is not a nationally accredited Level I trauma center. Better off flying them to Philly to a real trauma center.

(Yes, I know you were being sarcastic, but I couldn't resist).

but how are we going to get this male patient out of the house without compromising his spine while keeping his airway open all while he is leaning against a wall?? ooh and your the only person on scene........

Cloning...

or if only there was a place spewing copius amounts of radiation that I could visit to gain superpowers?
