Do you get your ACLS & PHTLS certifications through a medic course, or is it like BLS CPR course that you taken separately?
 
You can do it either way. Most (but not all) medic programs will get you the initial certification. You'll still pay for it though. :)
 
Most medic classes here you take the ACLS as part of your Cardiac portion and you receive a card at the end of school. No program here that I am aware of does a PHTLS.

You can do either of these as a separate stand alone class. The programs here only charge the cost of the card.

Depends on the school as stated above.

My program we did ACLS, PALS, ITLS, ITLS Access, NRP and CBRNE.

I believe now they just do the first 4. I think Access is hit or miss depending on availability of instructors since its not super relevant.
