My medic partner saw this and didn't know what to think at first, what's your take on it? IFT 78 y/o female ICU for respiratory failure x1 week ago
Vitals B/P-118/88 manual pulse-108 RR-20 98% 3LPM via NC temp 97.6 skin-normal color temp moisture no complaints of pain

Without the availability of a 12-lead and by your pt assessment I'd call that a sinus tach.

That looks like Sinus Tach to me as well. And the STach isn't all that fast. By description, the patient sounds to me to be fairly stable. Without more to go by, I don't see much that worries me yet.
 
Rate looks like it is 150. 2:1 A flutter is also a possibility.
 
I agree with a possible 2:1 atrial flutter, but a 12-lead would have been nice.

Looks to be a sinus tach in the 130 range. 12 lead would definitely help though
 
It looked like 2:1 flutter to me at first but the premature beat in the first rhythm strip was a happy coincidence that proves it is not. You would see the 'hidden' flutter wave after the QRS.
 
Sinus tach. Probably RAE also.
