Have you been drinking tonight, sir?

I used to carry a small notepad but very quickly decided it wasn't the best idea because of the contamination issue so i carry several note cards with me then tape it to my leg if i need to work on the pt. (check vitals... etc.) and once in the ambulance i will switch to the run sheet. Usually only thing i write down are pt's name, age, DOB, CC, and a set of vitals.

Usually write on my glove unless I have multiple patients, then I found that a couple of pieces of three inch silk tape on the wall or bench seat work great.
