That’s a fair point, that said there was a transition between Coumadin and xaralto/eliquis during which I’m pretty sure everyone was put on plavix for everything..

It also depends upon medication compliance, whether you have a surgical patient, and exactly how much you want to shift INR - Xaralto is long acting compared to warfin, as are most of the others - and they're also said to be somewhat more difficult to reverse as compared to warfin.
