    NREMT-P from Texas

I'm in Austin too. Work full-time as an attorney here and volunteer as a paramedic less than an hour away.

    Community Paramedics revisited

Am I the only one who sees a disconnect when you have the same person dealing with the two extremes of our patient population? It would seem that the skill sets/interventions/mindsets for dealing with the critically, acutely ill patient are by no means the same as dealing with low acuity...

    Texas certification vs NREMT

Texas has 3-4 different ways to maintain Texas certification/licensure. Maintaining your National Registry is but one way.

Off duty at a restaurant, you witness a bari+geriatric fall

All kidding aside, with the bruising, I'm wondering if she's on anticoagulant therapy. Couple that with the slurred speech, and you've got a great chance for a CVA or some sort of head bleed. Keep her still, get as much history as you can, and wait for the on duty crew to show up. Unless, of...

What kind of analgesic do you use?

Where I am right now, we have fentanyl and our benzos are midazolam and diazepam. We're supposed to be getting lorazepam next and possibly getting morphine back. (Apparently several of the local cardiologists still prefer it.) My last service carried fentanyl and demerol for pain management...

    Jewish EMTLIFERS

You don't have to be Jewish to like Mel Brooks, but it does help.... And regarding History of the World II, I'd be willing to be one of the Jews in space....

    Jewish EMTLIFERS

I'm also Jewish. Heavy on the ISH part, mind you. In other words, very non-observant. But I like Yiddish humor and Mel Brooks moves.

    Best states for EMS?

Texas is, without a doubt, one of the best places to be in EMS. There are some excellent services here and we don't have a heavily regulated set of statewide or county protocols. The medical directors for individual EMS services have the ability to set the protocols for their services. The...

No RSI- Teeth clenched from Head Injury-WWYD

I don't disagree there. I just like to throw options out there. I think the "devil's advocate" role came with the JD.... <_<

No RSI- Teeth clenched from Head Injury-WWYD

I am most likely mistaken, probably even flat-out wrong, but isn't there some study that indicated that the risk from nasal intubation (or inserting a NPA) in a patient with a basilar skull fracture is actually fairly minimal? And if nasal intubation is contraindicated, I seem to remember...

Worst names for an ambulance service...

There are over 200 private ambulance services in Houston. My favorite is Medi-Taxi. Dark blue ambulance with the yellow/black checkerboard banding around it.

    nremt-p study guide

Two suggestions. 1) Google "Jon Puryear." His NREMT-P prep course is phenomenal. 2) Consider investing in a SAT/ACT test prep book. Don't worry about anything other than the section(s) on test-taking strategies and tips. Standardized tests have the same strategies regardless of the...

    Chest Pain Diagnosis

Just to clarify, ACS/myocardial infarction doesn't always present with chest pain. It's sometimes a bugger to identify. I have seen severe cases even present with nothing more than nausea. A medical director I once practiced under gave the following advice regarding ACS. 25% of patients...

Things you would like to do to your rig but would never get away with

Two things: 1) A speaker playing music like the ice cream man. 2) As an attorney and paramedic, I want a sign on the ceiling above the head of the stretcher that reads, "If you can read this, you may be eligible for a cash settlement."

It's something I'd consider in this situation. I wasn't there, so it's hard for me to say. To me, the medical term consider means that you use your clinical judgment to make the best possible decision for your patient, factoring in all considerations such as your skill level, available...
