Another issue I've seen is that the for-profit ALS services' competition with each other compromises patient care. I have heard of a few incidents when a BLS crew was told that no medics were available by the ALS dispatch center run by one ALS company, when another company's ALS unit was available.

Just wondering which states besides NJ (if any) only have hospital-based ALS. I've found it to be a bit cumbersome having this restriction as it seems to reduce the availability of ALS and increase on scene times. Anyone have experience in NJ and another state as far as how this works/doesn't work?

It must have been hell for them to set it up and contract with local municipalities and the agencies that have the certificate-of-need.

As others have said, NJ state law mandates that a 'provisional' EMT cannot fill a crew and must be under the supervision of another EMT at all times. My squad has one that can bring a backboarded pt. to the ambulance for off-road incidents. It also has various IC/MCI supplies (tags, tent, water bottles, etc.).

I agree that EMS & LEOs don't always respond compassionately / competently to psychiatric emergences, especially suicides. However, as others stated, there's essentially zero chance of additional 'special' personell being utilized for these situations as you're suggesting. When someone calls 911...

Not sure about pay, but a fair number of first aid squads are hiring per diem EMTs to fill volunteer coverage gaps. PM me for a few Essex county area squads that are doing this.
