Working overnight on a BLS unit

It depends on the company, so we can't help you too much. At my job, the BLS car does a fair bit of psych transfers and ED discharges at night. Sometimes an urgent transfer from a nursing home to an ED (when they don't call 911).

Is there a doctor (or other medical provider) on the plane?

I was actually on a plane with a medical emergency a few weeks ago. On descent into San Jose, the flight crew made an announcement about it, however it was evident it was already under control. I was almost disappointed that there wasn't a "Is there a doctor on the plane" announcement and I...

    IV Solu-Medrol

I had my -P-school medical director explain this to me when I was in school... It still rings true, and I still follow his advice. When you walk through the doors of the ED, it's going to take the better part of 20-30 minutes for the staff to perform any meaningful interventions on that patient...

    Ultrasound in EMS

It's an awesome tool with a LOT of promise for EMS. Yes, there's the FAST (and RUSH) exam. But that's just the tip of the iceberg. It's a great way to get peripheral IV access, "listen" to lung sounds, and even more - we can measure things that matter - like ICP or preload/afterload/fluid...

Medics, what was the hardest part about transitioning from EMT to medic?

I think the biggest part is what others have mentioned.... You realize that you no longer have the safety net you had in having a preceptor who could take over if you screwed up. First code, I realized I was the only medic on the scene, and I needed to coordinate a host of other providers to...

I'm just glad Dale volunteered :) As @PodMedic once said.... "I have a face for radio"

    Football EMS Standby

@hops19 , it's awesome to see someone like you seeking out our feedback :) From my days doing high school football standbys, things were always vague, and there was a lot of hope that EMS wasn't needed. College was much better, partially because the school has a large AT education program, and...

How to do a better patient assessment?

I think @DEmedic hit the nail on the head. The best assessments involve just talking with the patient, developing rapport, and getting the information you need. SAMPLE and OPQRST are semi-useful mnemonics, but too often I see an EMT or student, face in the clipboard, asking short questions and...

DC FEMS Medical Director writes brutal resignation letter

Actually, I think it was.... They had the authority for months to do it. I think they finally made it happen as an effort to "do something" after her letter.

DC FEMS Medical Director writes brutal resignation letter

I've heard a lot of folks who "used to work at Grady"... I kinda wonder what their turnover is.

Does TranScare still exist in NY? Seems like they are shutting down everywhere.

    veteran medics

I'm with @akflightmedic... I'm a FTO at my one job, where I am also the medic with highest seniority. Honestly? I'm at a point where I'd call myself an EMS Journeyman... I'm comfortable, but not a renowned expert.

    Recert by exam

I did it again this cycle. I'm 3/3 for recert by exam. Given that my employer doesn't pay for my refreshers, its WAYYY cheaper than taking time off from work and paying for a refresher.
