    C-Spine rearing its ugly head

They ought to know what NEXUS/CCR is. It's basically a standard of practice now, but I'm sure you know the fact that it's in the protocol doesn't mean much around here. I'm not in LA anymore, but the whole even-though-it's-in-the-protocols-I-don't-think-you-should-have-done-it thing still...

    C-Spine rearing its ugly head

Yeah I don't get it man. This is like a universal Southern California thing to have hospitals be kind of unofficially at odds with random EMS protocols - even the rare progressive ones. I suppose that isn't all that unusual, but the near universal rejection of EMS c-spine clearance by hospitals...

    C-Spine rearing its ugly head

No CT capabilities on site. Couldn't really manage the patient there either.

    C-Spine rearing its ugly head

Not really. They asked me why I didn't do it, and after I answered proceeded to do it anyway. Pretty standard fair. Agreed. Problem is everyone around here interprets that part of NEXUS rather conservatively.

    C-Spine rearing its ugly head

Shame on all three of you! As soon as care was transferred in the ED the resident ordered her to be placed in a c-collar. I hope you learned your lesson.

    C-Spine rearing its ugly head

Here in wonderful SoCal we are in love with c-spine. It's like our main form of treatment in EMS. Even though we have a spinal clearance protocol in pretty much every county, it's just one of those things that hasn't quite entered the realm of cultural acceptance. Oddly, and perhaps ironically...

Accuracy/reliability - Manual BP vs. Automated.

Generally I agree that the NIBP is a better option due to convenience and accuracy is probably better than manual sphygmomanometers since they're rarely calibrated or maintained properly. The only problem with them in my experience is their accuracy tends to drop off once the patient becomes...

    UK Paramedic heading to CA

This is no doubt true, and I've always known things to be generally better in NorCal vs. SoCal. It's not all that surprising though given cost of living in the Bay. Median income for San Jose is the highest in the nation at ~$70k. Median home price is ~$500k for Santa Clara County. Both of...

    UK Paramedic heading to CA

Yeah that's pretty much the entire developed world outside the US does it this way. Because of the failure of the American education system, you basically have to repeat high school for the first 2 years of your undergraduate studies.

    UK Paramedic heading to CA

It's difficult to say as UK and US undergrad degrees are quite different. Our degrees have "prerequisites" which is basically a list of courses (I think you would call them "modules") that have to be completed prior to starting the main degree coursework. For nursing there are a host of basic...

    UK Paramedic heading to CA

Ah, well this is most surprising! But a good surprise no less. This is a good mentality, because you'll have no other choice! Just to be clear, paramedic licensure counts for very little outside EMS here too. If you end up going RN, PA, MD/DO you're going to be starting completely over. Your...

    UK Paramedic heading to CA

$100k?? Best paying private service I know of is AMR-Ventura and it's nowhere near that. I would be quite happy to be wrong about this though!

    UK Paramedic heading to CA

Well looks like I'm a bit late to the party here and terrible_one and Anthony have provided you with some really good information. I will, unfortunately, add to the bad news. Sorry, mate but it's only fair to you. First of all, here are some links to some organizations that you need to become...

    US Paramedic to Queensland Paramedic

I was in Australia rather recently, and was fortunate enough to do a ride along with QAS. The crew I was with said there were two Americans working in their particular division and gave me their contact details. One replied to me, and from what she outlined it is quite a taxing ordeal. Not...

Will birth certificate work as 2nd ID?

Pretty sure it has to be a photo ID. I used my DL and passport. If you don't have a passport, then a student ID should work.

Greatest Contributor to Burnout in EMS

The usual contributors (i.e. low pay, poor/unhygienic working conditions, incompetent management, sleep disruption, frivolous calls, etc.) certainly go a long way in exacerbating my frustrations with EMS. However, the single biggest contributor to my desire to no longer be a paramedic is...

    800 numbers on Ambulances......

The public health system. EMS is part of public health is it not? And why is the extreme notion of complete social breakdown the standard? Just because something likely wouldn't result in a complete dystopian situation, doesn't mean it's still not harmful or problematic. Sure, exclusive...

    800 numbers on Ambulances......

As I said elsewhere, they have full authorization to do this; and, given that these are institutions they have the capability to maintain an internal system of quality assurance, oversight, and so on. It's just pure confusion to liken such scenarios with individual, private citizens randomly and...

    800 numbers on Ambulances......

The only number the public needs is or should have in their minds when they have an emergency is 911. I actually disagree at the outset that healthcare is even a commercial industry, but that's neither here nor there. Advertising to healthcare facilities/institutions is a different matter...
