Do we still have to get the medical examiner's certificate signed by the doctor on top of the DL-51? Anybody know? I was told on this thread that the doc needs to sign off just the DL-51.

I see. My PCP is not registered by the National Registry of Medical Examiners, so I will need to find a examiner who is registered, and from my understanding... only bring a DL-51. I was just asking to make sure so that I wouldn't have to go a second time to the doc just because the DMV forgot...

I have found a list of doctors approved by the DMV. My only wonder is, what do I bring for the doctor to fill out? Just the DL-51 or do I also include the medical examiner's certificate that I found on this thread, but never given at the DMV?
