Schedules and crew rest policies

Currently working for a newer smaller air medical company. I am trying to collect data from any and all companies as far as:
1) What company do you work for (optional)?
2) What schedule do you work (12’s, 24’s, 48’s, 96’s and the associated days of)?
3) What is your crew rest policy?
4) Do you use a crew resource matrix app or a RAM score to determine level of fatigue (if so what company do you use)?
Thank you in advance for any information. If possible would you be willing to email me your policy?
 
2. Most areas are on a 24-hour schedule for the clinicians as a CAMTS requirement however we do have some very select areas that are able to do 48-hour shifts. Rotations for the 24-hour shifts is XOXOOOOO.
3. Crew rest can be called at any time by any crew member. The length of time the base is out depends on what the crew wants. It could be an hour or 6 hours however 4 hours is the most popular option.
4. Whenever a clinician has a shift that is extending beyond 24 hours we have a risk assessment form that is completed that gives us guidelines on what to do based on our score. If we get a high score then the clinician is unable to extend their shift and must have 10 hours of rest before returning to duty. Other scores allow us to go 8 hours, 12 hours, 16 hours, etc. It's an internal company-based risk assessment.

12 hour shifts.

3) What is your crew rest policy?

No specific policy regarding rest on shift , but a minimum time mandated between leaving working and starting a new shift.

No but this is briefed daily. There has been some discussion of a more formal tool.
 
FAA Part 135 rules for Air Ambulance Crewmember require 10 hours of consecutive rest in a 24hr period with a mandatory 24hr continuous rest period every 7 days. These rest periods also apply to maintenance personnel.
