Recert by exam submission

Does anyone have the email for NREMT to send the recert by exam form? Last recert cycle I mailed it in and they lost it, so I ended up emailing it and I cannot remember the address.

Call them and ask. Is this that supplemental skills verification form? Or is it the application to take the test? If it is the later that should all be done online and submitted electronically.

Everything is done; this is the skills verification. I've done this once before but I cannot remember the email they provided for me to submit the documents to

Yeah I mailed mine the one time I did recert by exam. I say call them and ask.
