Ideally, the top ten inches of an old D or E cylinder including the stem valve, and a regulator/tank level yoke. Does not need to work, better if it doesn't. Just want to teach people how to use a yoke. A fixed rate regulator would also be good, again, I can make it cosmetically acceptable.

Know of any sources? My local ones won't do it.
 
You might try contacting whoever does the hydros in your area, see if you can pick up a cylinder that failed, then just cut off the top of it. Of course that leaves you trying to find a yoke and regulator as well. Maybe check with a service company to see if they have any junkers.

Why not pick up a cheap M6 oxygen cylinder on eBay for $12 and spend another $10 or so on a medical oxygen regulator. The bottle is tiny, and regulators are cheap on eBay.
