It is the class of medication that should be the relative contraindication, not the dx of pulmonary htn or erectile dysfunction for that matter.
Oh, I completely agree. The problem is that most EMS protocols talk about ED medications, and not necessarily the class of drug.

ASA is also given to PTs with chest discomfort in a cardiac emergency. It is a vaso-dilator and works similar to NTG. ASA and NTG can be used together.

Directly from Brady Emergency Care 11th Edition:
Medication Name:
1. Generic: Nitroglycerin
2. Trade: Nitrostat, Nitrolingual
Indications:
All the following conditions must be met:
1. PT has CP
2. PT has Hx of Cardiac Problems
3. PT has Rx for NTG
4. Systolic BP greater than 90 to 100 systolic.
5. Medical Direction gives OK for this PT.

Contraindications:
1. PT has hypotension or a systolic BP below 90 to 100.
2. PT has PR below 50 or above 100. Follow local Protocol.
3. PT has head injury.
4. PT is a infant or child.
5. PT has already taken max dose. (Normally 3)
6. PT has taken sexual enhancement drugs in the past 72 hours.

Dosage:
One dose. Repeat in 5 minutes, if less than complete relief, if systolic BP remains above 90 to 100, and if authorized by MD, up to a max does of 3.

Medication Name:
1. Generic: Aspirin
2. Trade: many available

Indications:
1. PT c/o CP
2. PT not allergic to ASA.
3. PT has no Hx of Asthma.
4. PT is not taking anti-clotting medications.
5. PT has not other contraindications.
6. PT able to swallow.
7. Medical Direction gives OK for this PT.

Contraindications:
1. PT unable to chew and swallow.
2. PT allergic to ASA.
3. PT has Hx of Asthma.
4. PT has known GI issue.
5. PT has known Bleeding disorder.
6. Medical Direction Gives OK for this PT.

Dosage:
162 to 324 mg (two to four 81 mg tablets of chewable baby ASA). ASA does not usually need to be administered more than once in the early treatment of cardiac problems.

Even though the book does state the above please be sure to check with your local protocol on what you have available to you. Colorado goes by the National Registry information listed above for the administration of NTG and ASA.
