Hyperkalemia induced failure of pacemaker capture and sensing.
There's a great podcast that reviews the latest, best, evidence for the treatment of hyperkalemia. Emcrit is aimed at emergency medicine physicians, but plenty of medics listen to it.
Some of the main points - calcium first and often, kayexalate is of dubious value (and may be harmful), and albuterol has great clinical evidence to support it. Much better evidence than sodium bicarb, in fact!
