What are the main uses for giving D5W to a pt.?
 
Mixing it with medications that aren't supposed to be in NS or LR.
 
Cool, do you have some examples?
 
Pre-hospital it's just used for mixing. Some drugs go better with D5W and cant be using with NSS or LR. Usually a 250mL bag of D5W is used to make an Epi or Ami drip here. We have Dopamine and Lidocaine pre-mixed.
 
Amiodarone IV Infusion

Cardizem

Dobutamine (NS as well)

Dopamine (NS as well)

Epi for an Epi drip

Fentanyl for a Fent drip (NS as well)

Insulin (NS as well)

Labetalol (NS as well)

Ativan (NS as well)

Versed (NS as well)

Morphine (NS as well)

Nicardipine

Tridil (NS as well)

Levophed

Nipride



Those are just in my guidelines. Obviously, many exist.
 
Meds, NPO patients (although this is usually D5 1/2NS), patients on insulin drips, treatment of hyperkalemia, the list goes on and on.

Off the top of my head, I think a couple of antibiotics don't do well in NS or LR.
 
Is D5W preferred over NS because it is more hypotonic and will 'pull' the drug into the individual cells better? If not why would it be preferred over NS or LR?

D5W will facilitate the entry (i.e."pull") of the medication into the cell better, but it has to do with cellular metabolisim, not the tonicity of the solution.

Have I told you I'm just a little jealous of you yet? If not... I'm :censored::censored::censored::censored:ing jealous. Can't wait to become a medic
 
D5, or D5&1/2, 1/4 etc are also used as maintenance infusions
 
We use it for ketamine (analgesia/induction) and amiodarone drips

Adrenaline drips we mix in a one litre bag of NS

For hypoglycaemia we use 10% glucose IV or PO
 
Amiodarone and Dopamine are the only things we use it for. Never gave either so....
