De-cock the "Rescue Rick" system and institute tiered response with many substations for the second tier. Accept that many patients will be non-emergenct and accommodate that, including protocols allowing transport declines and teaching techs more public and general health on top of their very basic trauma skills.
How about this: convene a feisty Grand Jury (not Congress, they just bury things or people) and tear open the termite nest that has jacked the cost of an ambulance response up to the price of the down payment on a car.
