It's one of those things that you'll find differ from ED to ED. For example, for hospital A, I can just do a down and dirty, "it's medic 104 with a 30 year old female with nausea and vomiting x3 days. Vitals are stable, got a line, labs drawn and 8 of zofran on board. See you in 15."
Hospital B is a different story. They have a form that they fill out on every radio call in. So, I asked the charge nurse for a copy of the form, laminated it and keep it in my clipboard. I can write on it with a sharpie and wipe it clean when I'm done. And they LOVE my reports.
Find out what the hospitals want. And then do that.
