Are they still mandating that the EDs find hours into for homeless people before they will discharge them?

In the case of these vaccines manufacturers were allowed to run multiple trial phases concurrently to speed up development. Phase III trials are really about better understanding clinical efficacy and when to use a drug in practice once it is Phase IV (which is just post market surveillance)...

    COVID VACCINE - The Megathread

I’m scheduled to get mine this week. So far we have paid any of our staff who contract COVID and have been working clinically to stay home and quarantine, in several cases for 5-6 weeks. I wonder if staff decline vaccination will they still be allowed to collect quarantine pay or get cut off...

I have become soft and like indoor plumbing too much to want to go back to the streets for anything more than an occasional transport or facilitating one of the specialty programs.

    COVID VACCINE - The Megathread

Hesitantly yes, I’m supposed to get mine early next week. I’m concerned about the long term efficacy, at this point we don’t know that it can prevent transmission for more than a couple of months. I’m concerned that like influenza and other single strand RNA viruses that good efficacy only will...

To play a bit of the devils advocate the field is nowhere for a new grad nurse to start to practice. The goal of nursing school is to develop a broad base of understanding so that they can then safely and efficaciously develop skills and knowledge to care for more specialized patients. This is...

What could I have done differently for this call?

You know plenty well that the package inserts don’t necessarily reflect the most current practice and are based on the early and limited clinical trials and typically only updated if there are serious concerns post market. While the package insert states the duration of analgesia being longer...

What has changed since you started?

Unfortunately there isn't a lot of consistency when it comes to naming internal hospital emergencies. Often smaller (whether by hospital size, total trauma volume, average acuity of trauma volume, or a multitude of other factors) find it easier to have only one alert rather than several. These...

What has changed since you started?

The argument I’ve always heard is that an 18 under pressure provides similar flow to a 14 by gravity. That being said I can apply pressure to the 14 as well. Some pressure infusers cannot keep up with a 14, cordis, or RIC when pressure infusing crystalloid. That isn’t the case with blood...

What could I have done differently for this call?

I didn’t suggest we stop using toradol, I suggested that we follow the current evidence and maintain current practice in medication administration. There is a large amount of evidence that there is a analgesic ceiling at a 10-15 mg dose IV and that higher doses do not provide more relief. I...

What could I have done differently for this call?

I have seen several cases of AKI after a single dose of toradol in patients who were previously healthy. A higher dose of toradol does increase the risk of kidney insult.

What has changed since you started?

Two large bore IVs (with the expectation of 14s if possible) for adults is still an expectation in some areas. It is here for trauma Red/1 and code whites. For the patient who needs massive transfusion a 14 is going to provide faster flow over an 18, especially when you consider how much more...

What could I have done differently for this call?

That story is highly embellished at best. The literature shows equal analgesia between a toradol dose of 10, 15, and 30 mg in adults when given IV. Standard dosing frequency is every 6 hours if given as repeat dosing IV. Higher and more frequent dosing are more likely to result in adverse...

Swans are very rarely used, I have seen one in the past ten years. There is a lot of literature which has demonstrated swans increase LOS and complications, so we only really use them when we absolutely must. These studies go back over two decades so it surprises me that IBSC and AACN hold on to...
