Which is better: public or private EMS?

The only thing is that I would be hesitant to plan an entire career around working for that one private contractor, due to job security. The municipality isn't going to go anywhere. Although layoffs can happen, the muni job is still a much better bet for staying in one place for the long term...

Which is better: public or private EMS?

My county FD could take in one trillion dollars, and we would still have the same budget. We don't get to keep it, it all goes into the general fund. If the BOS says our budget gets a haircut, the budget gets a haircut.

    Gracie Jiu-Jitsu in Herndon VA

He lived in CA at first with some of the Gracie brothers, then moved to Utah to do his own thing. He later identified that there was a strong market for BJJ in Northern VA, and moved here. I either take a morning class on the way home from work, or I drive an hour in to take back-to-back classes...

    Gracie Jiu-Jitsu in Herndon VA

I've been rolling at Pedro Sauer's Gracie Jiu-Jitsu academy at 360 Herndon Parkway in Herndon VA. The building is in the same row as the HQ for Physicians Transport Service, a local IFT company. It's self defense oriented for the lower belt levels as opposed to a sport Jiu-Jitsu focus, and...

Which is better: public or private EMS?

If you want to stay with one employer for your entire career, I would go with municipal. With municipal, you can typically get a DB (pension), or a DB/DC hybrid at the least (half pension, half 457). Most privates are only 403b or 401k. Just look through this forum, there are many posts about...

    Promotion to leadership

Have an open competitive exam, where an outside company administers/grades the tests, and controls the rank #'s of the promotional list. The company must promote from the list in order, and not skip anyone. Have pertinent pre-reqs to sit for the exam, perhaps leadership classes, statistics...

Who's doing the NY spinal protocols already?

http://thesocialmedic.net/2015/03/nyc-remac-eliminating-backboards-from-spinal-immobilization/ Got my answer for the removal of the LSB. Makes sense, since those thing cause more injuries than they prevent. We just try to stay with the collar and low semi-fowler's, or have the pt. refuse the...

Who's doing the NY spinal protocols already?

Suprising - studies have shown that when the patient self-extricates, they will guard/minimize painful movement when they move from the seat to the cot. More undesired movement occurred when providers were applying the KED, or when they were moving the patient in-line with the LSB inside the...

What call do you absolutely hate?

A couple of nights ago I had another call that drives me up the wall - the lonely 65-80 y/o lady calling at 0130 that lives alone and has absolutely nothing wrong with her, just looking for attention. At this time of night I'm looking to turn the call over in the shortest time possible without...

Who's doing the NY spinal protocols already?

What is the new NY spinal protocol? For a few years we've been able to go with a just a collar, or nothing at all depending on pt presentation. We can also have them self-extricate instead of doing. KED or sliding a board under their butt and inevitably contortions them (defeating the purpose)...

What call do you absolutely hate?

Panic attacks really wear me out. I've never run so many panic attacks before I started working in Northern Virginia. People at work in an office. People calling from their car on the side of the road or highway (get that a lot). College students in class. Panic attacks during an incident with...

What is your opinion of pain management?

We have Fent 1mcg/kg, max single dose of 100mcg, max total of 200mcg, and we can call for more. 1/2 single dose, but same cumulative dose for pts 65 y/o or older. We carry 400mcg on our rigs. We're now able to incorporate Ketamine into pain management as well. Burn pts. and pts already on opiods...

Austin Travis County EMS hiring Paramedics and paying them as Basics

You're probably right. I could do over 100 or more hours of OT, and they'll still hold me if I have no relief. What I do is take the forced OT at 1.5x comp time, and just take another Sat or Sun off. I used to not do that, but I work so much OT that I have no problem playing hooky to take a day...

Austin Travis County EMS hiring Paramedics and paying them as Basics

Once in a while we get holdovers during the week, but for the medics in busy stations, they're regularly in jeopardy of getting held on Sat. and Sun. Our relief is one for one, then it goes to others in the station, then it goes to the battalion holdover/recall list, then it goes...

Austin Travis County EMS hiring Paramedics and paying them as Basics

From what I've read, it sounded like mandatory holdovers were a frequent thing, not just on-call once a month with a rare holdover due to no relief. Are you saying that besides the once per month on-call, it's rare to be held over for for an additional 12-24 hrs after your regular shift due to...
