    How to fix EMS

Why should they be? What have they done that demonstrates a willingness and/or capability to implement such an agenda? At least the NAEMSP and NASEMSD were the ones who actually developed the EMS Education Agenda for the Future...way back in 1996. Fifteen years later, we're only just now barely...

patient right to demand ambulance transport

Do you happen to know the relevant legal references for this in California? My attempts at researching this haven't yielded any results.

    Best states for EMS?

If you somehow manage to get a now pretty much extinct FF/PM job with LAFD (they're the only ones who pay anything like that excluding a handful of very small other departments, and some departments up in the Bay Area). And even if you do, this gravy train's heyday is coming to an end. People...

NCTI is a medic mill with Fortune 500 dreams, stay away from it. Do the real deal and go to the University of New Mexico or the University of Pittsburgh

    How to fix EMS

Under the scheme I suggested there would no longer be EMTs. All existing EMTs would have to upgrade to something more or less equivalent to what is currently referred to as AEMT (formerly EMT-Intermediate) which would itself then require an AS degree and carry some corresponding scope of...

    Pediatric Scenario

Like you said, the absence of adventitious breath sounds is not sufficient to rule out anything, especially not in a crying/fussy/squirmy kid. It could sound more or less "normal" but there could still be consolidation or thick secretions obstructing airflow or impeding gas exchange...

    Pediatric Scenario

Prodrome of URI? So, to summarize we have a febrile, tachypneic, toxic appearing child with some increased work of breathing. Has he hit all the appropriate developmental milestones? Is he consolable? Good muscle tone? Percussion sounds? Feeding habits? Urine output? Lacrimation...

    Best EMS Organizations

Yeah I know, but I'm considering them as private since the services are not provided by the local government directly. Thanks man!

    Best EMS Organizations

Yes. Looks like I C/P'd MedStar into the wrong spot, thanks for that. I consider EMSA and REMSA private since the former contracts with Paramedics Plus and the latter is an NPO.

    Best EMS Organizations

I know a similar thread was made in the past, but I was hoping with the moderator's permission this could be turned into a sticky thread. What I was thinking we'd do is make a comprehensive list of the EMS organizations with the best reputations as far as working conditions, clinical...

    How to fix EMS

I'm well aware of this. The entire healthcare system needs reform. Because they're short-sighted. Raising the standards of the profession will require short-term sacrifices which will be easily offset by long-term gains.

    How to fix EMS

I guess that's why this isn't a business right? It's just not designed for market principles to function. If they lie to you about the nature of the patients you're supposed to care for why continue to do business with them? You might even have grounds for a lawsuit against them.

    How to fix EMS

I would be fine with that. Whether or not there's a political will to do so isn't really the point, although I agree with you there would be significant opposition (not from the NAEMT which has essentially no lobbying power, but definitely from peripheral organizations with like interests)...

    How to fix EMS

If they're unwilling to provide you with the necessary information, then you can't in good conscience do business with them. This is also in your own self-interest, looker as I'm sure you wouldn't want to appear in court one day with just this as your defense. In LA County (and pretty much...

    How to fix EMS

You do realize in LA County you're not supposed to take that call at all right? You're actually supposed to do some degree of caller interrogation, and find out why this patient needs to go to the ED when there's supposedly a physician on the premises. Then you're supposed to advise them to call...

    How to fix EMS

It isn't a business. It's a professional social service where the profit motive has no place. This is why it is the responsibility of government and possibly philanthropic organizations. Not the market. It would be a tough fight, but I'm confident that with growing public awareness of the...
