ED and Security

After two years as a basic, I've never really run into this before. We had transported a patient and had just brought her into the ED. The exterior door has a numeric keypad as I guess most do. My partner was doing his report, I went out to clean the truck. When I was done I headed back in and there was a guy hanging out by the door. He said "how do you get into this place?" and pretty quickly I figured out he was a drug seeker. I said "you need a code" and blocked the keypad while I entered it.

I went in and of course he followed behind me. I think he bothered a couple of nurses and I'm sure he was ejected eventually. No real harm, although the place was mad-house busy. I asked my senior what I should have done and didn't really get an answer. I could have tried to order him out, I could have shouted "I NEED SECURITY HERE" or I could just think "hey not my problem" but that seems wrong.

What do you do to stop drug seekers or others from piggybacking you into the ED?

Similarly one night I was in the ED and someone else's psych patient bolted for the exit with the medic yelling "hey hey hey! stop him!" which I was not about to do.

Civilians aren't allowed in our ambulance entrance or parking area unless they rode in with us, then they go straight out into the waiting room. I would have told him to leave and definitely wouldn't have opened the door with him standing right there.

Hate to say it but that was a bad call dude. How would you live with yourself if he piggybacked in on you and attacked a staff member or a patient?

If someone piggybacked on me into one of the ERs here I'd be in a boatload of trouble. If you were my partner you'd have gotten a stern talking to for that one.

If I was the medic asking for help to stop the pt and you let him run on by a supervisor would be hearing about it. What if the psyc pt was going after a nurse and she was asking for help? Oh yea, its not your job so just let it happen right? :rolleyes:

I would have lived with myself by saying "nobody told me otherwise". Or I would have said what I tell my wife when I can't divine the unstated: "sorry dear, I left my ESP in my other pants". I have no idea what the hospital policies are and where people are "allowed" and in fact I went back outside to see if there were any "no unauthorized personel" signs. There were none. I agree it was a bad call, that is why I asked my medic and with no real answer I'm asking here.

Answer me this: If people can't just walk through triage and into the ER what makes you think it's ok to let someone piggy back off you through the ambulance entrance?

I'm not trying to be a ****, I know it sounds that way though but thats downright dangerous. Maybe I'm jaded because I live and work in a city where violent crime has quadrupled in the past couple of years, who knows.

For the one who followed you in you can tell him he has to use the front entrance. I know that the E.D.'s here have a metal detector everyone has to go through. If some one snuck in with a weapon they might start looking at why. As far as the psych, stick out a leg and trip him.

I think it's pretty clear I think I was wrong or I wouldn't have been asking here. The point of my question was really what should I have done instead of letting him in. But I'll add another part of scene safety to this. It was 2 AM and I'd rather not be alone out there with this dangerous murder either. I'd rather get myself inside and call security if he insists on following me in.

Also, I don't think you're being a ****. I'm agreeing with you more than you think. And I can accept that your ED is a lot more dangerous than mine. So point taken.

Just tell him that he cant go through that door. If your worried act like you forgot something in the ambulance and go there.

Point taken. Personally the way I would have handled it would be as flight said and tell him he needed to use the front entrance. If he refused I agree on opening the the door and scooting inside and notifying someone. Our charge nurse desks are all right at the entrance doors so it's easy for us to be like 'ma'am this gentleman refuses to go through triage and is trying to follow me in' and they will take care of it pretty quickly.

It was in response to the statement I made. No they aren't but they definitely aren't a stand up human being trying to sneak into an ED at 2 am...

Maybe he was looking for the entrance, hell he even asked someone who opened up the door for him.

We've had people try and get into our entrance at the different hospitals. We direct them around to the main entrance. I'm most of them are either lost or just stepped outside for a smoke/phone call but I'm still not going to let them in. My biggest concern is that they are visitors people don't want, abusive parents/partners, non-custodial parents etc.
