What Qs do you ask when taking a history?

It was originally used to assess pain mostly I think, but it can be adapted for most chief complaints.

Onset
Provocation/Palliation
Quality
Radiation/region
Severity
Time

You may also see OPQRST-ASPN

The ASPN stands for Associated symptoms and pertinent negatives.
