Tiered response or all out response?

Do your local EMS calls all get highest level care, or added on as needed?

I'm more and more intersted in the total Australian model.

Too bad it probably belongs in the Exotic EMS or whatever they named it section.
EMS tagged along on the fire dept tails here because the FD's had the infrastructure to get it moving fast and under government control (to get the DOT money I suppose). Private services have had a reputation for attracting and hiring "outlaws", newbies, and part-timers (medical students, off-duty military or fire dept, etc).
I suppose a comparison might be the peanutbutter paradigm: you can spread it thin so it covers lots of people (interesting visual mixed metaphor) but it isn't as substantial; or pile it on thick but it can only be shared so far and so many ways (and costs more per person too).
