I was speaking with a former co worker from NSLIJ, a NYC 911 prehospital provider. I suggested that I'd like to possible work there again per diem, to keep my skills sharp. I'm not seeing enough high acuity pts here. Problem is, I'm about 270 miles from NY. It wouldn't be worth it, even though it's in excess of 30/hr.

I'm thinking that I can find the urban environment that I'm looking for at Richmond. It's only about 70 miles from home, with 12 hour shifts, PT available. The site says that new medics get 15/hr. I've got 4 years as a medic currently. Protocols are on the site. Does anyone have any info in RAA?
 
Richmond is a great system to work in... I think you'll get the urban stuff you want from NYC without the stairs. There just aren't any walk-ups that have more than 2 stories.

The protocols are progressive, the survival to discharge on cardiac arrests is 25%- or something like that. Dr Ornato, the medical director, is a really hands-on guy who is always trying a different modality for treatment of cardiac patients.

There's plenty of inner-city environment there, a typical night will have a couple of shootings/stabbings. There are about 6-8 trucks on the street at night, so your chances of getting good trauma are relatively high.

There is the trouble of system status management; there are no stations, you just sit on assigned street corners waiting for your calls. I like to work, so I work hard, and in a decent shift, I could get 6-8 calls in 12 hours. If you like to work, go there. It's fun, you pick up great experience, and run lots of calls. There isn't as much non-emergency stuff as there used to be, at night it'll be rare that you have to do a non-emergency call, and in the day time it'll be rare that you do more than one in a shift. If you don't like to work, don't go there though. You will be busy. People who work in a busy system and complain about it all day make me sick.
 
And dude, you considered commuting to NYC from Stafford??? That's nuts.
