Well I had a cardiac arrest this weekend, could not get the tube. Huge amounts of emesis, even chunky at time. Couldn't see the cords, tried a combitube but felt like I would have to really force it. I went with oral airway and BVM.
Did you think about a bougiee? The anesthesiologist that I primarily worked with for my intermediate suggested for those intubations that are relatively blind because of the body of habitus that he puts the tube anterior and inferior of the epiglottis and levers (the tube hand!!) the tube into the trachea.
One consideration is that with all of this vomitus, consider that positive airway pressure is causing aspiration. What do you mean about having to force the combitube? Like it was too large for the patient?
Is that your only rescue airway?
Please note, I am not questioning your decision making, You go with the airway you can get.
