If I were to start my own business providing EMTs for standbys in NJ. What would it cost for a medical director? Would it be a one time fee? Or a yearly fee?
 
Depends on the deal you can get with the Doc you talk to. I wouldn't expect a one time fee and done though. For continuing labor, one expects to pay a yearly salary of some sort.
 
It depends on the MD. And how you contract.

I know of some useless ones who are half retired and provide "direction" by way of signing off on things for 10 different services.

Your asking a professional to extend your providers his license and assume some liability for their actions. That's probably not going to come cheap. For some good physicians look at NAEMSP docs in your area or leaders in your local academia/community setting.

I'm going to wager you'll be coming off some money though
 
I figured it wouldn't be a one time fee. It probably would t be worth it than unless I could get a lot of standbys.

Also, since NJ has so many volunteer squads that do tons of stand by events, it may be hard to start something like this. MONOC's control over other major events may also be an issue.
