What would you include in the HPI of a seizure pt.?
This is why I ask....
History of Present Illness:
Pt. wife states pt. has history seizures, pt. wife states current seizure started roughly 5 minutes before calling 911. Wife is unaware of anything that might have caused seizure to begin and states pt. was watching tv when it started. Pt. seizure currently affecting left side of face (twitching) and left hand (jerking). Pt. seizure lasted approximately 46 minutes using 5 minutes prior to call as start time. Document a thorough HPI of seizure patient. This looks as if you didn’t attempt to interview the family member. I know you better than that. You just need to write it down.
