You call the medical director (immediately), he initiates any immediate action necessary (calling the hospital if you didn't notice the error until after the call) and starts the process of investigating the error to determine if there are ways to prevent it happening in the future. There is no penalty for self reporting, and near immediate dismissal if it looks like you lied or were trying to hide something.
