Flaws in collecting the road injury statistics!

I know this article is for New Zealand serious road crash injuries.
I was surprised to read they take the statistics from Police reports, while on scene.
Do you know who records them in your Country?

I also found it interesting as to how they record a minor to major injury. Re the comment on fractures.

Most States has data input from State Department of Safety that is submitted by LEO. It includes multiple data as well most states have now require NEMSIS data collection of PCR which is compiled from State EMS.
