Way back when, I had difficulty recalling all the details. Much less trying to work in a new system that had/expected different radio reports. So the simple solution was a "Post It" (TM) type note pad. Easy to keep in a shirt pocket, disposable. Personally I never wrote the name. A quick HPI note with vitals and times and or doses. Even on the busiest days when I was 6-7 charts down, I could complete charts by referring back to the stickies.

Over the years, I tried 3" tape on the leg. Actually worked fairly well. However when really busy, the tape would double up and become illegible. So back to the stickies I went.

But now, it has become second nature. Whenever I meet with students that are having difficulty with radio reports, I revert back to stickies.

Just jot the following down on a few sequential stickies and your ready to go:
Unit #
Age-Sex
HPI
V/S
Tx (treatment)
ETA

A quick, down and dirty report and if we to chart later, a quick reference.
