Good afternoon,

I have received an e-mail back from McCormick and they're asking for a H-6 form from the DMV. A Google search and a bit of reading later I find that there are two kinds--one I can get online for $2 and one I can get for $5 which they say is the official document.

1. Can I use the online print out or would I have to request an official driver's license record and send the money through the mail/wait for the document to come to my address?

2. If I were to need the official $5 document, could I go to a DMV and request it and get it on the same day?

They're asking me to come in this coming Monday and I want to make sure I have everything prepared. Thank you in advanced!

You need the $5 one, the online one is an unofficial report. Yes you get it typically the same day, if you've had several violations and actions that make it over two pages they print it out for you right there.
 
Yes, you need the $5 one. That's universal not just for McCormick, but pretty much any and all ambulance jobs (and I'd imagine any other job requiring a copy of your H6) The only thing you get if you get the $2 online version is wasted money, I once tried to use it and was told that it wasn't the right one and I still had to go to the DMV for the $5 one. The $2 printout is a completely different format, doesn't look anything like the official $5 one.

Yes you go to the DMV, tell them you want an H6 printout, get your number, wait, go to the window, it's been a while since I've had to get one so I can't quite recall 100% for sure, but besides having your Drivers License in hand, have a copy of your proof of insurance as well just in case, you pay them a 5 Dollar Bill and they print off your form right there at the window. Basically, wait for a few hours (if you go at lunch or 5 pm or other peak hours, if you have the time, I recommend going in the middle of the day during the week when everyone else is at work, but your mileage may vary on that tip *shrugs* bring a book or a tablet with full battery....I've been in and out in less than 20 minutes....and have waited 4 hours as well) and spend 30 seconds at the window and done.

The H6 is printed with the date you went to the DMV and got it, and pretty much every company I've seen has said you have to submit one that's within 30 days (so if you're actively job hunting, you can get one, and make copies, but once a month is up, it's no good and you'll have to go get a fresh one).

I think it said in the e-mail that it had to be at most a week old for McCormick so my only hope is going as soon as I can tomorrow. They also mentioned that I have to take a test for the Thomas Map Guide--any pointers and tips on that? Never went over that in EMT class so I'm not entirely sure what to expect
 
Thomas Guide is a commercially available map book. Specific to LA/Orange Counties, owned by Rand McNally now I do believe, it has no ties to anything EMT or EMS specific beyond being an easy to use way of getting around that's been popular before GPS was popular and in everyone's pocket. Of course EMT school isn't gonna cover it, anymore than they'd cover how to check the oil and tire pressure on your ambulance, or operate the fuel pump at the gas station to fill it up.

You can look up tips on Google or YouTube better than I can explain here. But it's not hard, just an analog version of whichever app on your phone you use to get around, except you have to flip the pages instead of swiping lol (Well that and it doesn't pinpoint your location or your destination for you, you have to do those manually.....which you'll have to do to work for McCormick, besides having a class in new hire oruentation/training, during your FTO period you'll be required to map to every call using only your Thomas Guide, no GPS allowed. If you can't map from your location to the call address (dispatch will give you the map page and grid for the call, they're typically, though not always, either on the same page or an adjacent page to where you're at, but anyways, if you can't successfully map your way to calls, all of them, using only the Thomas Guide, you don't pass FTO)

I'll try to familiarize myself with looking at maps a bit more before the date then, so I don't completely mess that up then. Thank you for the prompt replies!

Passed all the tests. Interviewed with Lucas right after patient assessment and he offered me a job on the spot. Thought it'd be spaced out a bit more but I'm not complaining! I start orientation soon.
