How do EMTs know if a DNR order form is "real"? I don't want to just rely on a page that has fancy words and a bunch of signatures. What does a DNR look like (specific color, any specifications)? Are all of them set up the same?

Some states have specific prehospital forms. Some don't. Some states will allow something as simple as a "DNR" or "no code" order in a patient's medical record (for example, nursing home patients) to be a valid DNR for EMS providers.

Yeah, follow your local protocols. We can accept POLST, physician orders and DNR medallions.

For the most part, if you are unsure if its real, appears out of date, or a bulky packet of advanced directives to sift through. Call medical control

This is a link to New Jersey procedures for DNR and it also has a copy of a pre-hospital DNR.

Generally we are looking for the pre-hospital DNR and/or POLST order. New Jersey allows use to accept DNR pracelets, anklets and necklaces as valid DNR orders. In healthcare settings, we can except other documentation of DNR if it appears valid. Same rule applies for out of stand DNR.
