CA Ambulance DL - Medical Examination where to go?

I'm getting closer to be a fully licensed EMT-1/EMT-B in California.
Passed my finals in training and already made my appointment for the NREMT written exam.

Reading "check lists" of what else needs to be done I'm about to tackle DMV and get my "green card" - I downloaded, and started filling out my part for the medical examination as required by California state law.
And that's where I noticed that there's a couple of sections, in particular about eyesight, hearing that I wonder, can my regular physician that I go to for my annual physical and any other flues, etc. test me on those?
If not, I think I need to see a medical professional that fulfills the DMV requirements and can do all those checks for me - so that's where my question comes up:
Can anyone in the East Bay of the SF Bay Area share with me which doctor they went to for that? Any input is appreciated so that I can verify if that doctor also would be covered by my health insurance....

Any MD or DO can fill it out as well as PAs and NPs working under a physician. Additionally DCs can also fill it out if they're "clinically competent" to do so (stop laughing... no really... stop laughing).

Just make sure they are registered through the DMV. I had that problem with my primary when I first got mine.
 
Occ Med might be best as they do these all the times, your PCP might not be used to the form IE. my PCP did not check the box that I was not blind and I had to go back.
 
I know a doctor who specializes in physical exams and such, only if youre willing to drive to san jose. PM if interested

Not sure how it works exactly... My primary couldn't do it because he was no longer registered through the DMV.

I guess it is possible, was just not aware of this requirement.
