Here us some " hot topics"

outcome based procedures

use of hypertonic saline in the field

Do paramedics "really need to intubate" ?

Degree Paramedics versus non-degree Paramedics

Use of "synthetic" blood in EMS

Permissive hypotension in shock syndromes, versus fluid therapy

The "Golden Hour" myth

Field termination of resuscitation of pediatric codes

Use of L/S on private vehicles and number of death & injuries r/t this

Training versus Education, in EMS

There are several others.... do a literary search, such as in Journal of Trauma, JEMS, Emerginet, etc...

Good luck,

The golden hour is a myth?
Well... there is still a lot of "evidence" missing from it... it is one of the points of EMS that hasn't changed in years, and seems to be considered a sacred commandment, as if it was handed down from "on high."

Very true... the research conducted by Dr. R Adams Cowley the founder and author of "Shock Trauma".. pushed and pushed hard to justify trauma centers.. some of the research team was Dr. Red Dukes, (sound familiar) etc.. although, appeared credible at the time... many, now wonder about the credibility as well, as if it is even true?

Like for so many years, we have taken things at face value like trendelenburg (which has shown, not to work), fluid resuscitation (which actually harms the patient) and even oxygen therapy at high flow rate is being scrutinized ( hard proving it has changed any outcome, in non-respiratory cases.

That is why evidence based medicine is so crucial.. "Why we do, what we do"...other than, that is the way we always done it...

Did you ever wonder how the "always done it" way came into being?

Start with a cage containing five apes. In the cage, hang a banana on a string and put stairs under it. Before long, an ape will go to the stairs and start to climb towards the Banana.

As soon as he touches the stairs, spray all of the apes with cold water. After a while, another ape makes an attempt with the same result--all the apes are sprayed with cold water.

Turn off the cold water. If, later, another ape tries to climb the stairs, the other apes will try to prevent it even though no water sprays them.

Now, remove one ape from the cage and replace it with a new one. The New ape sees the banana and wants to climb the stairs. To his horror, all of the other apes attack him. After another attempt and attack, he knows that if he tries to climb the stairs, he will be assaulted.

Next, remove another of the original five apes and replace it with a New one. The newcomer goes to the stairs and is attacked. The previous Newcomer takes part in the punishment with enthusiasm.

Again, replace a third original ape with a new one. The new one makes it to the stairs and is attacked as well. Two of the four apes that beat him have no idea why they were not permitted to climb the stairs, or why they are participating in the beating of the newest ape.

After replacing the fourth and fifth original apes, all the apes which have been sprayed with cold water have been replaced.
Nevertheless, no ape ever again approaches the stairs. Why not?
