    While I agree freelancing can cause difficulties, if something goes down and I am there, I'm still going to render aid that is appropriate to my level of training. Once appropriate manpower is on scene, the operations officer can decide if he/she wants to still utilize me.

    Oops, forgot to add.. the research I've done indicates the first EMS model was published in a "white" paper in 1966. Shortly after, the fire departments began training the first generation of paramedics.

    To me, I would have to agree. If I had a DNR. I would not want you to bag me. If I am suffering, maybe some pain management or sedation. No heroic measures. Beyond that, it falls within what is specified within the DNR and what your state considers to be heroic measures.

    Change of clothes, toiletries, towel, night apparel, bedding if you have beds. Things to occupy time in between calls. Food if you can't or don't want to eat out.

    Google Translate

    We used it once for a German lady at the airport. It worked pretty well. But I find you have to use it in conversation mode and explain to the patient how to use it. Otherwise, the software never works right.

    When to (and not to) attempt resuscitation?

    I wouldn't because most of the time, the brain has suffered to much damage from hypoxia. One thing I like that we started doing is inducing hypothermia in patients that we get a ROSC in. Our data shows a definite increase in patient outcomes post resuscitation.

    CAB and ABC

    I think CAB makes much more sense. C= circulation. And part of that is assessing pulse. If you can't get a pulse, the rest is a moot point and compressions are a higher priority than breathing or airway. Again, this is evidence based treatment and we will do well to do it like AHA wants us to.

    Wound care

    Peroxide is awful stuff and should never be used to clean a wound. To flush a wound, I like to use bottled water. After that, I like to use a wet to dry bandaging techniques by wetting gauze with normal saline, cover the wound, then a layer of dry gauze over that, wrapped over with cling wrap.
