    What Gets ALS?

Huh. Never heard of that here. We routinely line and lab people while in the waiting room. Sometimes, we even get IV antibiotics or fluids running in the waiting room. The attitude here is if they want to leave, let them leave. But I guess we haven't been burned before...

    Psych Patient / run away

How did they make it up to the front of the ambulance and kick the driver out of his seat? Either way, if he is in the driver's seat and the rig is stopped, get out and dispatch for PD. If the ambulance is moving at a high enough rate of speed, buckle yourself in somewhere, brace for impact, try...

    Miracle Cure for Sepsis

So I actually read the full text of the paper. I mean, sure, the results were drastic but there are severe issues with this type of literature that limit any ability to draw conclusions from. First of all, this is a retrospective study, non-randomized. Second, the control group was for a 7 month...

Sinus tach with abarrency or VT...?

About how old? Pt. hx isn't really suggestive of VT, neither is the 12 lead. Rate is right around 100, what appears to be a fairly normal axis, and regularly regular. No classic sine wave appearance. Being post arrest, I don't really doubt that there are some pretty severe metabolic and...

    What have I done...

I don't think that it is fair to call that purely volunteer. That is more of a POC or paid-on-call system. While you may not have the run volume to support making bank off of it, you are still getting monetarily compensated for going on runs.

    Priority intervention?

But is Chicago so understaffed that the EMS vs. LE response times would differ significantly? The only place I see this making sense is somewhere with extended EMS response times due to chronic understaffing like Philly as mentioned, or maybe Detroit?

ROSC without defibrillation? ROSC with respiratory arrest?

Not anything except for epi during the arrest periods. Honestly, starting him on a rapid bolus of amiodarone or lidocaine probably wouldn't have hurt, but if I recall correctly, the doc was trying to get the family member to confirm requests not to work him any further, so it wasn't really a...

Can confirm. Was working a playoff Saturday when the Falcons made their run and the ED was quiet. Everyone in the city was too busy filling up the bars. Those who worked Super Bowl Sunday confirmed that it was one of the quietest afternoon/nights of the year.

    Miracle Cure for Sepsis

This is honestly the sort of study that doesn't need any funding. All the drugs are on formulary at every hospital's pharmacy already, and ready to be ordered. As long as it is done in a somewhat non-incompetent center, the hospital can randomize the study drug or placebo to sealed envelopes and...

New Intubation Method - looking for feedback from the field

My question is how is this device operationally different than a (specialty) laryngoscope blade with a flex-tip to help lift anterior anatomy and help provide a cleaner path to the cords?

New Intubation Method - looking for feedback from the field

If you watched the video, the subtitle said, "He tries to intubate the most severely injured casualty while stabilizing his neck." Based on this statement, an appropriate inference could be made that there are multiple patients and that triage had been conducted, both of which lead to a logical...

    Strange stuff on your truck

Yeah, @Grimes we have one too just like you described/the picture. Similarly, used for wrist fractures to aid in resetting in. It failed once in the poorest of ways possible. One of our ortho surgeon's kids broke his wrist playing football. Brought him into the ED and he wanted to do the...
