    Diversion

I don’t explicitly know who this is on this forum (although I think I can hazard a pretty good guess), but I wonder if the tone here is different because some of the regulars “knew”/knew him. I’m not necessarily implying anything, but if this same story about Joe Blow was posted on here, I think...

    Prehospital antibiotics

The ADRENAL trial result was discussed at the Critically Care Reviews conference in Belfast just recently. The gist was no improvement in mortality with steroid therapy, but decrease in surrogate clinical endpoints like ventilator days and I believe vasopressor requirements. There is a lot of...

    Student question about Tidal Volume!

To accurately determine a person’s tidal volume, one would usually expect the patient to be on a ventilator, or otherwise in an outpatient setting, by a pulmonary function test.

    Another mechanical CPR study abstract

I'd like to know what specifically this sentence means and whether they adjusted for this in their regression model.

    Sharing performance metrics

I think the biggest issue is getting providers to understand why you are showing them the metrics and what that means for the patients first and foremost. I'll echo what Vent said in saying that people often think you are trying to push your/some agenda in showing metrics and trying to get...

So do you cross the border to get him? Or does a Mexican ambulance drive up to the border and you drive up to the border and exchange at the border?

Doctor? Whoa, we have middle-of-the-road paramedics teaching ACLS round here parts.

    Identify LVO

Just so we're clear on some of the acronyms: LVO is large vein occlusion and EVO is endovascular vein occlusion?

Serious Question: Why Does Fire-based EMS sometimes produce such low results?

But this isn't true. The NEJM published a study about an increase in ED utilization in one state (oh, I want to say either Colorado or Washington) that saw increased ED usage after generous medicaid expansion. It is going to take a whole lot more than educating the public.

    CPAP for CPR

One would think, but similar early stage literature I've read on techniques that do this very thing seem positive. Specifically, I'm thinking of abdominal binding intra-arrest, which increases circulating volume secondary to shunting blood away from the abdominal aorta/inferior half of the body...

    Tourniquet with Femur Fracture

Was the wound still bleeding? If bleeding was controlled, there doesn't seem to be a need to TQ it. Especially with an open fracture, I would think you would see if there was any bleeding or not externally. I guess theoretically, internal bleeding could be a concern, but the leg would swell up...

A situation like that is my biggest professional nightmare. Especially from a hospital-based perspective, going from a non-trauma metro center to (will be going to) a level II that serves both a college town and large areas to the east, there isn't a day that a situation like this doesn't go...

    Ketorolac for pain management

It certainly should be a thought. There was a study in NEJM [http://www.nejm.org/doi/full/10.1056/NEJMsa1610524#t=abstract] a few months back that showed ED prescription patterns have a very significant effect on long term opioid use. Between low intensity and high intensity providers...

    IV Pumps

Isn't this...isn't this how it is everywhere? We don't even worry about getting a pump back. We just transfer the patient to wherever they need to go and get another pump from our clean supply room back in the ED.

Stryker chair 6252 stair pro Accidents?

I still don't think you are understanding this. If the chair was at the top end of "tolerance", then that means it is within acceptable manufacturer specified ranges. The chair does not have a mechanical problem.

What would you change about your protocols?

I don't know about this one. The general malaise patients are often pretty sick people that end up getting admitted, especially in the older patient population.

    Lightning Strike

But you have to look at the reason behind why we would black tag them. We black tag them because in an MCI, do the most good for the most people. So, that means don't overcommit multiple resources to a person with a high risk of mortality (traumatic arrest) if that means that other potentially...
