DC FEMS using Privates for low-acuity calls

How do you know a call acuity until you get there?

You respond to 40 y/o with chest pains and coughing

It's absolutely ridiculous but something I've wondered about.

You respond to 40 y/o with chest pains and coughing

Strict triage says black tag your partner. However, he's probably viable. I would ask for any help I can get. You can't go anywhere so work the arrest and wait for another ambulance even if it's coming from afar. If you have more than two arms, get your original patient on oxygen. What else...

    Calling 911 vs. Driving?

Thanks all. Unfortunately I'm awkwardly biased. Too many people I know have driven themselves in for CP or post-syncope. When I did my EMT ride alongs, I saw a man brought in to the ED in cardiac arrest. He was driving to the hospital with a CC of CP when he arrested. He was dead. I'm so...

    Calling 911 vs. Driving?

We are always told to call 911 instead of driving to the hospital, and I teach this as well. Does anyone know of any studies that compare survival rates on these things? Perhaps urban vs rural, etc? A quick search gave me a ton of websites but few research oriented ones. Thanks.

    Blow-By O2 or Recovery Position?

What? In 1985 they were giving tobacco smoke through the rectum for drowning?

    BLS vs. ALS?

EDIT: Apparently there is an ongoing discussion in the ALS forum.

    BLS vs. ALS?

One of my old instructors sent me this article today. http://sciencelife.uchospitals.edu/2015/10/13/advanced-ambulance-care-increases-mortality/ I'm curious--what are your thoughts? The obvious concern is, since BLS and ALS ambulances don't respond to the exact same call, but rather, are part...

    Closest APPROPRIATE facility

4 hours was his words... But yea it seems a bit odd.

    Closest APPROPRIATE facility

I think this is a really interesting topic. I'm curious as to when (if ever) it's no longer worth it to make a long distance transfer. At a recent drill I met a paramedic from a rural area. He said that he would often have a 4+ hour drive time from the scene of a trauma to be within radio...

    Medical Director / Licensure

Not saying they should, I like the idea of having some sort of uniform standard, but was curious as to where the arbitrary demarcation in the law came from.

    When/why do you guys intubate?

Out of curiosity, it seems like King is the preferred airway on this forum. In what little my instructors have told me about these airway devices, it would seem that the Combitube is the best as a backup airway because it is blind insertion, but unlike the King, if you manage to hit the trachea...

This is the key piece of information I was missing.

    Medical Director / Licensure

Makes sense. I just wasn't sure what the deal with the state license was. With EMR or CPR or First Aid, its a card printed by the teacher of the class essentially. And for those first aid tents, the medical director is apparently happy with that. But to do work on an ambulance... yes, you have...

    Medical Director / Licensure

No I don't. A quick google search makes it seem someone got in trouble for something?

    Medical Director / Licensure

Interesting. Back in my first responder training, I was told that if Dr. ABCDE (our medical director) gave me emergency phone instructions on how to do some sort of surgery with makeshift tools, I should do it UNLESS I believed him to no longer sound of mind.

    When/why do you guys intubate?

I'm curious, and was hoping that some of you ALS providers could fill me in on this. When/why do you intubate patients? My understanding is that concerns is the airway (duh) and of getting air into the lungs, not the stomach. Also to prevent the chance of vomit interfering with the airway...

    Medical Director / Licensure

This is a question just out of curiosity. What is the role of state licensure of an EMT/Paramedic if an MD has to be overseeing the operation, and anyone acting under the MD does so under the MD's orders? As an example, in many places throughout the country, Red Cross does first aid stations...

    Stroke?

Was just curious to know what was actually happening, obviously you err on the side of assuming a stroke. Good point. If we were a BLS ambulance I'd do that. As a first response unit we had to wait. Pupils were pinpoint, but it was bright daylight, so... Family members advise "she is...

    Stroke?

Dispatched as a foot team to the corner of Main St & Boring Ave for a 78 year old female "not feeling well." Upon arrival you find a huddle of people surrounding the lady sitting on the sidewalk hyperventilating. You begin taking to her, getting on her level and turning off your radio to try...
