    Severe hemorrhage

Its closer than you think. Junctional tourniquets are good. Depending on where the wound is though the best course of action may be to go knuckle deep in the wound, find the artery and manually compress until you can get a clamp in there.

Moving a Trauma Patient in severe pain without medications

Im gonna go a different route then the people above me. If he is hemodynamically stable (Shock index, BP, MAP, cap refill) then you need to treat pain prior to movement. It shouldnt take long to deliver pain medication (IV, IM, IN) along with proper splinting of affected areas to reduce pain...

Versed after Narcan for OD patients?

Reasons these patients are combative after narcan administration 1. They are hypoxic 2. They are hypercapnic 3. They have a sympathetic surge 4. Mixed use of drugs. If you treated hypoxia and hypercapnia before giving narcan alot of it can be avoided.

    Tranexamic Acid

@TransportJockey That sucks, from my understanding theres not much data supporting the 2nd infusion over 8 hours but it is all about how close to the time of injury it is administered. If we are talking about the same LVL 1 center, then ive seen them throw our blood products in the trash just...

    Tranexamic Acid

used it like crazy at a previous agency. Its not something you are gonna be able to tell how well it works. If they got close to meeting the criteria we just gave it. From my understanding (and i may not be up to date) there is no real harm in giving it when its not needed. So we just had a...

@CWATT a 50ml TV wouldnt be too low for this kid do to the hemodynamics, i think it comes out to just above 4ml/kg which some protocols recommend for hemodynamically unstable patients.

    Pediatric scenario

So the best i could tell was since they were using adult vent tubing, and they were on pressure control, the vent could not sense how much volume it was giving. Too much deadspace for too little volume, so sometimes it wasnt sensing any air return and i guess miss calculating when it did. The...
