    Germany and Netherland EMS system

Hi Carl. I am a Dutch paramedic in one of the larger cities in the Netherlands. You can PM me. I'll be happy to help.

    Coumadin toxicity

This interesting case has been presented somewhere else on the internet, and is worth sharing with you guys: A patient is brought in by EMS with an ´´allergic reaction with a swollen purple tongue and bruising of the chin and neck´´. The ED physician identifies the situation as coumadin toxicity...

Stupid **** your partner has said/done

A medic with that attitude wouldn´t survive his probation period with us, as nobody tolerates that kind of behaviour.

What is your most ridiculous call?

We responded to a call where a man arrived at his home in the middle of the night, with both his balls hanging outside of his lacerated scrotum. His wife (deadly cold and not amused at all) states that the man had been out drinking with his friends and when he arrived, she noticed that his pants...

Stupid **** your partner has said/done

I once had a partner who attempted to splint a lower leg prosthesis, because he thought it was damn obvious that the 45 degree angle suggested it was broken anyway, and he didn´t need to cut clothing and expose the leg to see it. He was pissed at the patient for not telling him too =)

Stupid **** your partner has said/done

But I´ve had patients where I really couldn´t tell....and it felt inappropriate to say the least to ask...so are you a man or a woman?

    Seizure secondary to ETOH abuse

I do agree on considing conversion disorder in this case. However I would not exclude the option of withdrawel seizures based on this patient´s young age as we see many young people these days that start binge drinking at 11, 12yo, and by the time they reach their mid-twenties they fit the...

    Infectious disease call

This patient was not on any medication. She started antimalaria profylaxis, but didn´t finish the prescription. We know one thing for sure: there was plenty of malaria in that region. But we don´t know what else. Onfortunately I don´t have a follow-up as the receiving hospital is not one that we...

    Infectious disease call

You know what, dispatch only mentioned Africa and we considered whether we should enter with only gloves based on the available info, or also put on a facial mask. If this happened two years ago during the Ebola outbreak, we would probably have stayed away and asked for a specialty team to...

    Infectious disease call

I considered yellow fever as well. And with yellow fever also Ckikungunya, which seems to be widespread as well in the tropics and have a similar symptom presentation. There where no major clues indeed: no coughing or sore thraot, no breathing problems, no GI-problems, no blood, no bruises or...

    Infectious disease call

We put malaria high on our DD, at the same time realizing that there must be many other diseases out there that will present with the same symptoms.

    Infectious disease call

This one is definately worth sharing with you guys, and I am especially interested in hearing from the guys that have previous experience in the (sub)tropics: Last week our ALS-unit responded to the following call: ´´Unresponsive 30yo female with high fever, just returned from Africa.´´ We...

    Vomiting Blood During Full Arrest

Indeed. What´s wrong with guys that see it as a sign of incompetency. No matter how experienced, you won´t always know, unless you are pretending to.

    Smoke inhalation injuries

Like most guys here, I enjoy this forum as a great place of learning and sharing of work experiences. I don´t feel the need to judge, or to be judged or to compete.

    Smoke inhalation injuries

After the cricothomy we´d be handbagging the patient with a BVM with PEEP, taking turns.

    Smoke inhalation injuries

I do like the idea of having a less distressed patient who is also in less pain. Absolutely.

    Smoke inhalation injuries

Noooo, I am not dying to give any meds. I can see that proper management of the A and the B is my number one priority... Okay, so I perform the surgical airway. Now I have a tube in and I can ventilate him, but for half an hour that he is lying on that stretcher, it doesn´t solve the other...

We use Versed as a sedation drug and anxiolitic, but it also has muscle relaxing and anticonvulsive properties.
