I almost died laughing after checking my patient's blood sugar.

I'm not sure I follow your logic?

Correct way for a blood pressure

I always take a manual, however I don't have another option, FWIW, I have yet to see an emergency department at any of the facilities I transport to use anything but an automatic, seems to be good enough for the hospital.

    Experience with Boston EMS? Anyone?

The department promotes existing paramedics. They also run a medic program in-house, but it is intermittent at best, there is a class in now, they have another year left, don't know what the plans for the future are. Most people attend programs outside the department, and then test when a...

    Experience with Boston EMS? Anyone?

I do. The pension is in what is called Group 4, same as Police and Fire, 32yrs/55 yrs old = 80%, also deferred comp plan if you choose, similar to 401K. Additional benefits are 50,000K life insurance at no cost from the Union, 15 paid holidays, accrue 15 sick days/yr, hazard pay, longevity...

    Experience with Boston EMS? Anyone?

Pretty good info, written exam, practical and oral interview, dept. physical and then 6 month academy. In the field portion you do 3-4 3 week rotations with another recruit and a preceptor that evaluates every call. If all goes well you will get a badge and a 1 year probationary period. All...

    Intraosseous access in cardiac arrest.

Interesting point of view Brown. It is without a doubt faster to place an IO than an IV in the vast majority of patient, it is relatively large (15g) access, hard to dislodge (unlike an IV) and is so easy a caveman (firefighter, volunteer even) can do it. Gathering a history should be more...

Do You Let Students Buy You Stuff?

A cup of coffee, yes, lunch? probably not. I have had doctors riding buy lunch, I'm more than happy to let them, but not students. Not saying your wrong if you do.

    chemical restraint

Anyone work in a system with a chemical restraint policy in place for combative patients? I would be interested in seeing your protocols and the system you work for. You can feel free to PM if you don't want to post publicly. Much appreciated.

    Physical "Restraints"...share your knowledge.

Just had a pissing contest with the associate medical director last week during her excited delirium lecture which was just before our 4 hour class on defensive tactics and hand cuffing. Apparently handcuffs are o.k., haldol not so much....<_<

Bariatrics Pt.! What would you do?

absolutely agree, however in the morbidly obese patient, picking up the knife should probably be the last resort, unless you are very comfortable in opening up a neck. Been there, done that, wouldn't want to do it again.

Bariatrics Pt.! What would you do?

I have a pretty good idea, I also realize that no improvement in 5 minutes doesn't equal RSI either. No one is suggesting "doing nothing", perhaps a bit of patience, PPV, TLC and pharmacology is what is needed, padawan....

Bariatrics Pt.! What would you do?

I honestly couldn't tell you, I'd guess a few dozen in excess of 500 lbs, maybe more, and have been lucky in having always secured it with an ET tube. We have a rehab hospital in my city with an entire floor dedicated to bariatric patients, never look forward to going there. As for supra...

Bariatrics Pt.! What would you do?

Really? 700 lb woman, do you realize how much tissue you are going to need to get through to cric her? Nightmare senario. Supra-glottic airway is also a bad idea, the amount of pressure needed to properly ventilate a morbidly obese patient is going to exceed the cuff pressure and quickly...
