    Hypotensive CHF patient

What was her normal BP? People get so stressed out about BP being low, without ever asking what the patients normal BP is. If this patient was my dad, yes that is a low BP (he is normally 150/90). If this is me, this is a little high (normal 80/40).

Is there a place someone can train a new EMT to drive a ambulance outside of a company?

What nomadic said works great. I didn't like driving the Internationals that my PT job has, then I drove a 20' Penske from NW Utah to San Antonio, and the Internationals are no problem now.

Do you need to stop at MVAs? What do you do if there is no need for EMS

That is always a tough question: I always stopped when I didn't have a patient on board; and a couple of times if it looked bad, when we did: but 1 of us always stayed with our patient. I put a tourniquet on a partially amputated leg on a motorcycle accident. When fire arrived, I got yelled...

    New Cardiac Scenario

The patient was me: and yes that is my normal BP; worries people sometimes. I was glad that I was sort of awake both when they took my BP and when the nurse came to give me the NTG paste. Although it would be kind of interesting to find out what happened (especially since I was already in...

    New Cardiac Scenario

Good Zebra; No all medical. No trauma. Turned out to be all stress related. Walk in to ED at 2130, walk out (of floor) at 1530 next day. CT stress test showed 24% EF (Ejection Fraction); but the cardiologist didn't believe it; so he ordered a bedside Ultrasound Echo which showed 64%. Slight...

    New Cardiac Scenario

2nd round VS: SPO2 and Cap approx the same, HR 80. BP 124/60. Chest and arm pain still about the same; at least until 1st dose of 5mg Morphine was given; along with Zofran. Patient felt like vomiting but Zofran finally kicked it. All of this is at the hospital; it was a walk in. 324mg...

    New Cardiac Scenario

P.E.'s were 10 day's post Knee Scope; and he wouldn't listen to his wife; so 4 days later after going to the surgeon to get cleared for work he went to the family doctor to find out why the Asthma was acting up so much. Doc was in the room for less than 1 minute, walked out, 2 minutes later...

    Medical alert activation WWYD?

I would call the police: bad thing in my PT job, rural area, PD may be 2 hours away, and may refuse to come for that. We have them refuse to come on a potentially dangerous run, "unless you are actively being shot at", So we force entry.

Longest shift, mood on shift and stress

No, PT job is 120 miles away from home (1 way) for me. we have a 3 bedroom 2 bathroom mobile home to stay in; and have 6 minutes to mark en route. So if you live in town you can go home. But you can get from anywhere in either town (same town, straddles state line), to station in less than 6...

Longest shift, mood on shift and stress

mgr22; yea PT job, Pick up shifts when we want. I had Use or Lose leave to burn at my FT job. (we work 48 hours on and 5 days off, {and we can trade shifts in the same pay week, so I can work Sunday Monday one week, and Friday Saturday the next week; which gives me 10 days off without taking...

    5 year old difficulty breathing

HR is normal for a child with a temp that high; remember the average HR goes up 10-15 for every degree of temperature: (Took me a couple of years of being beat by my medics to remember that). BP is good. If you have a child that doesn't do nebulizer's well, ask them if their parents have...

What is your most ridiculous call?

Not mine but one of the BLS crews I was working with one day: They got a run to transport from Hospital A to Hospital B lockdown unit: 23 y/o, male, transvestite, Vampire who was 19 months pregnant. Every crew that heard that made it to 1 of the hospitals to "help the crew" check the patient...

What is your most ridiculous call?

I have had a few where I can't keep from laughing as I call a report in; and I have told the charge nurse twice that I am bringing in a waiting room patient

Attempting to bridge the generational gap

Things I have learned from watching management and working hard. keep track of leave (vacation) requests. If you have to deny someone; write down who and what days they want. They may call in sick. if they do it a few times ask them for a doctor's note. If they keep bringing in a note from...

    New Cardiac Scenario

Here is one I ran into last Sunday: 57 y/o, w/m. Substernal chest pressure, radiating to left arm (alternating pain and pressure). Chest Pain 9/10, arm 8/10. Pale, cool, Diaphoretic. 12 leads at the hospital were ok, none to attach; they wouldn't give me any. Slight wheezing, lower lobes...

    Some specific DNR/DNI questions

The way I was taught is the DNR is for when they are actively dying; not when they break something,. But at the same time a patient can refuse an IV, even if they don't have an DNR. 1: DNR with broken leg. Treat the fx, take to the hospital, give pain medications however the patient will...

    Intubation and the unconscious

Lucky we have Versed and Fentanyl. I would give her a little of both; intubate, and drive slow (Ambulances do 75 and speed limit is 80) to meet a bird and fly them to a hospital. Closest hospital that will take this type of patient is 135 miles, and a bird would take 35-50 minutes on average...

    Patient centered approach at clinics

Must be nice; we get doctors that want the patient to go from the clinic in the town we are in (PT Job) to the hospital that some of them work out of. Which is 110 miles away. which is the closest hospital, but the only one that way unless we go another 150 miles beyond it. Most of the...
