    sinus arrest,pause,block?

Arrest and Pause are essentially the same thing. A block will have some sort of alteration in the PR segment and/or P:QRS ratio.

    Ibuprofen contraindications

And as mycrofft already alluded to, Reye's is not cause by ASA alone. Needs to be in the presence of a viral illness or a few metabolic disorders.

    Ibuprofen contraindications

Risks are probably over blown, but we'll likely never have a good answer due to the obvious problem of studying almost anything in children. Fairly common in the UK for infants/newborns to get NSAIDs as part of their peri-op pain regimen.

    Sodium Bicarb

His vent and acid base lectures are probably the best I've ever heard.

    Sodium Bicarb

That's because he's a smart guy. That blog was one of my favorite resources during school.

    Sodium Bicarb

Imminent herniation. Amp of Sodium Bicarb is crazy hypertonic. I'll never understand it, but I can keep all the Sodium bicarb I want on random unlocked carts and no one bats an eye when someone asks for it to be pushed. But I mention 3% saline and 50 people have an anger stroke unless I plan...

    Sodium Bicarb

Only one good reason to ever "push" an amp of bicarb. I'll occasionally toss a few amps in a liter and run it in with my maintenance fluids.

    Kidney Failure

Bodies are good at dealing with chronic changes. Wouldn't really bat an eye of a potassium around 6 - 8 in someone with long standing renal failure. 13 is pretty high, but again, body compensates pretty well, so when you're new set point is around 7-ish, 13 isn't terrible.

    Time spent on scene

I don't do crap on scene, with very few exceptions (e.g. 12-lead if warranted, cardiac arrest). And honestly that is double so for sick patients. Generally first question out of my mouth is "do you want to go to the hospital?" If the answer is yes I get them on the cot heading to the medic...

    kidney stones and a bgl?

I actually dislike most of the book. But there should be an entire damn class on the laws of the house taught in medical school. Especially the one I quoted above and "placement comes first."

    PA vs. Med School

So much wrong with what you said.

    kidney stones and a bgl?

"The delivery of good medical care is to do as much nothing as possible." This nugget gets forgotten all too often.

    Duty To Act Questions

    1. Call 9-1-1 as I drive on by 2. Call it in and continue on. I have a responsibility to the patient already in my care. 3. Render whatever aid I can while waiting for responders from the district.

    Cardiac Auscultation

I don't listen pre-hospital, in no way affects anything I'm going to do. In the hospital I generally listen in one area. The valve region is largely crap. There is so much transfer of sound waves in the chest that you can't reliably say where the murmur is just by auscultation (even adding in...

    What all do you see

    A terrible picture.
