Hey guys and ladys, I'm new to this site but have been thinking recently a change is needed. Ive been with Boston EMS as a high paid, well respected, busy, urban, 911 only EMT for 10 years. I am a medic working per diem at a low paid, disrespected, busy, urban private co. For 2 years as well. I'm looking for a new place to see. Would have to be busy, urban, and honestly I would love to go somewhere with a higher amount of penatrating trauma then we see in boston. Any ideas or reccomandations on how to begin my journey?

The most respected and progressive places that come to mind are in NC, DE, and TX companies. I believe they are all 3rd service too.

Any specific agency names or regions?

You won't really find tons of the penetrating trauma you're looking for in the systems terrible is referring to. Most of those systems are in fairly nice locales with limited impoverished areas.

I've been in the hood my whole career and would have a difficult time with little action.

Sussex County DE
Mecklenberg NC
Williamson County TX

Ya I had no idea the calls they run, I just picked the top agencies that came to mind.

I hear ya, I work in a pretty bad area myself (fire-based here) and love the clientele. All of the major "hoods" I can think of off the top of my head don't have really stellar EMS services, though. Not all are horrible, but none really stand out as great or extremely worthy of major relocation.

Maybe Denver health?

Thank you terrible and poetic for the help.. What about New Orleans or Memphis. And is there. Hospital based service for Atl?

Just looking up Denver health now seems promising. Any one skimming through this post have any knowledge of their system?

Does DHMC cover Aurora? If so, then I'd say they would be exactly what you're looking for. Shoot JeffDHMC a pm and he can tell you all about their system and call volume/types of runs.

Do a search there is a few threads about them. 3rd service that runs Denver county 4 10s, BLS fire, decent protocols.
New Orleans is 3rd service, be prepared for a large drop in pay though.

Rural Metro runs Auroura. Not a good system at all from what I've read.

Oh yeah, forgot about them. I remember the stories about RM and Aurora's fd now... Eesh, I'm out.

Denver Health runs city of Denver only. Aurora is its own separate nightmare of Aurora Fire and Rural Metro. Each surrounding area of Denver has its own system too of combos of Fire transport, non transport ALS, or privates. Denver Health has short transport times, double medic, BLS fire only on responses dispatches as "Code 10" (emergent response). Three great Level I centers in the city that have some great docs willing to teach if you have time as you turnover your truck. Do some ride alongs there first. Some people love it, and some don't. It wasn't where I wanted to work, but it might be your cup of tea. Penetrating trauma wise, it's still not necessarily the knife and gun club. As with any urban environment, you run a lot of nonsense with a large transient population. I will say the Denver area is freaking awesome though and it's a blast to live there.

I'll add to NC: Wake County, New Hanover County (also a nod to Brunswick and Pender counties which are in the area), Cumberland County, Surry County, Guiliford County, hmmm there are quite a number of good services. None of these pay terribly well, but all are progressive 3rd services by any standard. Most of those counties feature high call volumes, sick patients, and broad scopes. You're given fairly free reign to practice to the fullest of your scope. You're encouraged to be a clinician rather than a technician. Lots of great educational opportunities as well.

(If we were hiring I'd throw Leland Fire/Rescue in the mix)

If you're looking for a true gun and knife club, let me suggest Grady Hospital EMS in Atlanta. A poster recently referred to it as a slaughterfest more than anything. 4 10 hr shifts or 3 13s, shift diff, weekend and holiday pay, from what I can gather. I have no personal experience with the system, but for what you want it might be worth it to check it out.

Richmond Ambulance Authority is a pretty excellent place to work, and has its share of ghetto.

If your interested in overseas work in the middle east, I would check out these guys http://nationalambulance.ae/ there a pretty decent agency I use to work for. They just started a high volume 911 gig with lots of trauma from the crazy drivers over there.
