I think I F!@#$ up

I am in the us army and we were doing some training in hand to hand combat, I was working with one of my students on advanced throws and take downs. I was using him as a deminstration when I threw him and he landed on his shoulders and his chin hit his chest. He went limp. He was out cold.

he was unresponsive
puples normal
pulse and resp normal
BP 130/70
o2 sat 98
no crep or step off in the neck
no signs of head trama other than the unresponsivnes
good distal pulses
no motor sensery response
chest, abd, every thing normal
I put a c-collar on put him on a back board
got an IV started, I know I am only a EMTB but in the Army I am allowed to start an IV, and it is a standing order from the Army Docs
called the German ambulance 15 min later hes on the way to the hospital, time from trama to hospital 35 mins

this happend 4 days ago he is still unresponsive but stable. Docs dont know why he is not waking up. no swelling of the brain no neck injury.

I geuss my question is did I do any thing wrong with the treatment I gave??
 
Sounds like you did okay . The only things I might've done different would've been 15 lpm by non rebreather , being ready to assist ventilations if needed . It's every martial arts instructor's personal nightmare when one of your students gets hurt , and as much as we strive for safety , it does still happen . In my time teaching , I was lucky there were few injuries , non serious . It's okay to critique yourself , that's a natural thing . But don't let it intimidate you . You're training folks for life and death hand to hand combat that could save thier lives . You need to stay sharp and focused . I'll be praying for you both .

I wish I had o2 but they dont think I need it. I had to go and buy a bvm just so I would have one if I needed one. But thanks for your input I have another class I am giving on tuesday and it will be going untill friday so if I dont reply its because I may not have internet where I am at.
 
You did all you could do man, As Certguy said, 15L via non-Rebreather would have been beneficial, but you did as much as you could do for what you had available.

I hope your friend gets better, Keep up the good work!
 
That's a real Bummer, and I'm sad you had to go through it. There's nothing you could have done much different except to make the situation worse, which you didn't. That speaks volumes about your being able to keep your act together under what would have to be called impossible circumstances.

My prayers for everyone concerned.
 
If you are allowed to start an IV, could you have checked BGL? Obviously, that wasn't the problem since they would have found it, but given that it was an unknown at the time, a glucometer would be a good idea for an unconscious unknown.

If he has good respirations as you said, if the vitals held steady as you said, I doubt 15L NRB would have done much of anything, but it certainly wouldn't have hurt anything.
 
Thanks for your input He is doing a lot better now. He is awake now and out of the hospital, and back to work now.
 
what did they find out? you got me really curious about what his diagnosis was now
 
You did good with the equipment restraints that you had. You did not make mention of holding the spine while treating the patient, however i presumed it was done. As posted by by certguy monitor the ventilation closely as c3,4,5 keeps the diaphragm alive!! Aslo keep a closy eye on the airway, head injury patients (Which this patient may have had) often chunder. It can also take some time for an injury to show up on the CT scan, hence them doing repeat scans.

He had a concussion and a pinched nerve in his neck. He will need some physical therapy but all in all he will be fine.
 
wow, must have been some concussion to keep him out of it for 4 days, remind me not to pick a fight with you :P glad to hear he'll be fine
 
They said he was out for so long because of the way he landed on his head. There was some intracranial swelling, caused by him missing the padded mat we were training on and landing on the hard wood floor.
