History Questions to Ask For Peds

It seems like for pediatric calls regardless of the nature, there are a whole battery of questions to ask. Off the top of my head, these are things that I feel are important to find out:

Age
Weight
Medical History
Allergies
Meds

Recent Illness
Fever (if so, temperature)
Recent medications given

Infants:
Normal deliver
Term or Preterm
Complications

Eat, Sleep, Pee, Poo habits and amounts
Child acting accordingly per parents?
Child acting age appropriate?
Child color appropriate per parents?

I left out questions to ask for specific problems such as SOB/Febrile Sz's otherwise this post would never end. Are there any other general questions I should be asking regarding peds?

Those seem pretty good, and you will not have to ask them verbatim.

In fact, you shouldn't. Ask open-ended questions then focus on the replies for expansion or elimination, in time you will develop an internal script. WATCH how the parents act and react, LOOK around the environment when you make pickup, SMELL for odors like sweat, stool, vomit, ammonia versus urine, etc. Ask if the parents are OK, and any siblings.
