    Pt refusal

I guess this is just one of the many benefits of a well educated prehospital system. Yes, legally, we are required to transport anyone deemed a "patient" or they are to refuse. Even if you stubbed your toe we are required to offer transport to the hospital. Now, depending on where you work...

    Pt refusal

Yeah, this problem really needs to be addressed. Unfortunately, SNFs are requesting an ambulance because (generally) a physician has ordered that the pt be transferred to the hospital, often times without the pt ever being notified that this was taking place by the nurse or the physician. This...

    Race in PCRs

Forgive my ignorance, however this only highlights the need for a better system for race with our PCRs, or at least some clue as to what people belong in what category.

    Race in PCRs

How have researches found out that certain diseases are more prevalent among certain races? If no one records a patient's race that information is lost. It's also not just chronic conditions that certain races are prone to, it's also acute conditions like CVA and ACS. Also I recall reading a...

Can a small female succeed as an EMT?

I will agree with other posters, women are completely capable of performing this job. They also bring a different skill set on to scenes that some men lack. I have had several women partners and they have all been awesome, in fact I would rather work with a female than a male most times...

    Identity

I'm not entirely opposed to a system similar to the student doctors forum. If people go through a verification process an icon is displayed on their profile showing they are a physician or veteran or whatever. Actually I think it would be cool to have a similar system. I would only be okay with...

    Cores

The explanation I got was that it meant Cardiac Output, Respiratory Effort Zero. But it's always written down as "Cor" instead of "CORE". So who knows... Funny part is that I have heard that if you drive one hour south to Colorado Springs and say cor they look at you funny. I also like what...

    Cores

Cor in CO translates to cardiac arrest. Can also be called a Cor zero, don't ask me why lol.

EMT License in trouble of being revoked

I am not really sure what I just read but I did laugh through parts of it. So a pt with some wonky vitals and an "abnormal EKG" per a nurse should not have been transported? Really all I got out of that was 2 ricky rescue EMTs with inadequate training, education, and oversight had a mildly to...

The new reality of "scene safe..."

I would never ever do this. You may have training, but you lack some of the equipment necessary to keep you safe. Plus you have a partner who probably has no tactical training. I understand wanting to help but putting yourself, your partner, your ambulance, and the officers who may have to come...

The new reality of "scene safe..."

Several miles away? There really isn't, if you can see the guy I would call that the Hot zone, everything else is up in the air. Thats why I would suggest training some medics who can get into the hot zone safely and provide care while potentially under fire. That's a PD issue that they have...

The new reality of "scene safe..."

I have been thinking about this too. I think the delay the public is seeing with EMS getting on scene of these active shooter scenarios and other potentially volatile scenes is starting to become unacceptable. I think as more comes to light with the Aurora shooting the staging protocols for EMS...

Which services require spinal immobilization for penetrating trauma?

http://traumatoday.com/prehospital-spine-immobilization-for-penetrating-trauma.html This is kinda what you are looking for. Basically the extended scene times futzing around with a backboard that has no evidence it does anything increases mortality for penetrating trauma patients. And to...

Do I need a medical director?

Well you CAN, but don't expect to be a paramedic for much longer  I agree with NYMedic, beyond first aid you need to have a medical director. Hell, even then you should probably still have one for CYA purposes. And standing orders are written orders from your medical director, they...

Which services require spinal immobilization for penetrating trauma?

Not required here. I have never boarded a penetrating trauma, and I believe PHTLS recommends not immobilizing penetrating trauma even in the setting of deficits. I hope most agencies don't require boarding, but EMS and current medical literature aren't always in sync.

Different angle: why basic level technicians?

By every measure the US has one of the worst health care systems in the world. http://www.who.int/countries/en/ Compare Canada and the US, life expectancy is greater, and your chances of dying are smaller in Canada than the US, they also spend less of their GDP caring for themselves. I will...

MCI / Shooting-Bombing in Progress - Live Radio Feed

In Aurora it is standard for all incident traffic to go on a tac channel, typically tac 2. When there are large MCI's or fires tac 3 or tac 4 or some other tac channel is used. When there are large numbers of ambulances responding to an incident, the Denver Metro area has these regional...

MCI / Shooting-Bombing in Progress - Live Radio Feed

Keep the guys who responded to this in your thoughts/prayers. Some of them a having a really tough day today. I can't imagine having the phone ring at 1am and going on a shooting in Aurora (which is ridiculously common) and walk in to that disaster.

    5.11 Boots

I don't recommend 5.11 boots or belts, the leather tends to wear out very fast. The belt I had from 5.11 had scuff marks at the buckle within a week, and the boots have tears in the leather at the toe of the boot that won't polish away. That being said they are comfortable so I still wear them...

Why are Paramedics paid so little?

I am not a rocket surgeon, but I do think rocket surgery is a lot more complicated than having tools to work on rockets. I haven't seen a rocket surgery program, but I imagine that it is similar to an engineering program, which all require in the first two years: Calculus 1-3, calculus-based...
