Septal infarct > Flash Pulmonary edema > Code

Pt stated hasn't gone to Dr in a long time so no diagnosis on existing issues. He clearly should have been going to catch a multitude of serious impending Hart issues.

Septal infarct > Flash Pulmonary edema > Code

So to clarify, my monitor was doing automatic 12 leads every 15 mins. It just so happened to capture during a pulse check which was really neat and amazing timing actually. Pts pulse rate was actually 90s to 130s, and only dropped to 50 with the extremely low BP for a very brief period that...

Septal infarct > Flash Pulmonary edema > Code

Lesson from now on.... 500ml bags for everyone regardless to avoid fluid overload in future. Also with his hemoptysis I was initially questioning if this was possibly a volume issue as well as a pump problem ontop of that.

Septal infarct > Flash Pulmonary edema > Code

No need to apologise, I didn't take it that way at all man. Your good ☺️ There IS alot of play in our protocols, we have ALOT of freedom which is nice until something like this happens then it's like fuuujjck.

Septal infarct > Flash Pulmonary edema > Code

Soooo protocols states start with initial 250ml bolus, and " consider additional fluid bolus " if no still below 90 which it was. It states if no below 80 systolic which it WASNT except for one brief point to consider dopamine. Soooooo it isn't exactly clear on exact max for shock/...

Septal infarct > Flash Pulmonary edema > Code

We give 250ml boluses up to one liter. Checking lungs every 250. He did have a brief drop in b.p but overall trends showed a positive b.p reaction to fluids until 900 had gone in....

    STEMI Mimic?

Our stemi 12 leads are auto transmitted to all supervisors, management and up from there. So even if the monitor says stemi and it clearly isnt one its our companies policy to STILL to a physician consult to remove without any shadow of a doubt that were doing the right thing and not just...

What makes someone a "bad" paramedic?

Like point in case...Just showed up to a call......Here was my report: "Yeah this is mary, she's having troubling breathing. We couldnt get a pulse ox reading. If its respiratory she needs to get going to the hospital right now" And then cleared themselves which is against policy. And mind...

    STEMI Mimic?

Im in the habit of taking Serial 12 leads cause we often have **STEMI ALERT** Popup on our monitors without it being a stemi AT ALL......and we have to adjust leads slightly or just keep re-running them to get the message to go away....So thats sorta my way of eliminating the "error" portion of...

What makes someone a "bad" paramedic?

I've been an EMT doing everything actually. 911 on bls, als , a sprinkling of ift and also in the e.r. as a tech with an advanced scope and also a military medic... so I consider myself fairly well rounded. Fire doesn't have a REASON to act like our bls crews are a hindrance. They act that way...

What makes someone a "bad" paramedic?

Well put. This might as well be a thread of what not to do to be a ****ty provider. And so far I'm avoiding that. Privately I might have a worse opinion but I keep it private. Thanks everyone.

What makes someone a "bad" paramedic?

I agree with you. And I'm also aware of who are paramedics who work in the fire department and those guys are typically an obvious step above.

What makes someone a "bad" paramedic?

What makes someone a "bad" paramedic?

Excellent post. Great insight, I know fire despite being bls has been doing this a long time. And I've been an EMT for 6+ years so I'm well aware of their positions as well. I wouldn't say I'm burnt out. Just tired of multiple different engines in multiple different areas initially acting like...

What makes someone a "bad" paramedic?

And also.....i definitely do NOT act like a paragod . Hopefully i dont come off that way here. I know for a fact how little i know. i try to show confidence on scene but im the first one to ask for opinions or advice on things.

What makes someone a "bad" paramedic?

I fully agree with you guys that i presented that a bit poorly. So id agree with mgr22 that i AM bitter......No denying it. I DO want a team approach and an idea that we all work together but it comes to a point where your side is constantly criticized we end up doing some back. Mind you it...
